Competition Entry Forms and Help

NOTE: The 8th Edition of the ADAA Regulations (PDF 856 kb) is now available for download to view.  Members can purchase the rule book from ADAA for $10.00 (includes postage - see the Agility Shop).  The 8th Edition of the Regulations is effective from the 1st July 2006.

If you require Entry Forms there are 2 options:
1. Buy them in cheque book style booklets, see the Agility Shop for more details; or
2. Download the forms free of charge, in your selected format, using the links below.

Junior Members, please note that the Parent/Guardian Permission Slip needs to be sent to the club along with your entry forms if you are the owner of a dog entered, or are handling a dog entered.

In order to enter a Competition the Owner(s) and Handler(s) must be a member of, and all Dogs registered with, ADAA as at the close of entries (excluding late entries). Membership number and Dog registration number must be included on all entries. Please ensure that all Competition entry forms are signed by the owner, especially when the dog is to be handled by another person.  If a Junior Member owns and/or is the handler of a dog entered, the Parent/Guardian Permission Slip must be submitted with the entry form.

With the introduction of new Regulations there has come the need for new abbreviations when completing an Entry Forms.  View "Entry Form Help" (PDF 20kb) for these abbreviations.

Remember to fill in your Pairs Partners and Team Members on your entry form if applicable.
