Dog Registration and Transfer Forms

All forms will open in a new window of your Browser (to save, right click on the link and select 'Save Target As' or 'Save Link As').

*You will need to send a printed copy of the form confirmation page with your original signature and payment enclosed to ADAA. Prices and payment details are on the Dog Registration form.  Please allow 14 days after receipt of payment for processing of Dog Registration Applications or Dog Transfer / Change requests.  Any enquiries should be made a minimum of 21 days after dispatch of Applications. The applicant(s) may not enter an ADAA competition without having received their ADAA dog registration confirmation and registration details.

The applicant/member has read and understands the refund policy which is available at

 and by signing and lodging the form, the applicant/member agrees with and is bound by the refund policy.
