Forms for New Members of ADAA

These forms are for people joining ADAA for the first time.  Upon initial membership of ADAA (any category) registration of dog(s) is at no cost to the member.  Registration of additional dog(s) at a later date will be charged at a fee of $10.00 per dog.  You can visit our FAQ page to view the different ADAA Membership types.  View the Membership Fees here.

You will need to fill out a form for yourself, and a form for your dog(s).  If you are using the online form application you will be able to follow a link to register your dogs. If you are printing the forms to fill in, remember to print your form plus however many dog registration forms are required.

Please remember to include your email address if you would like 'Electronic' membership. 'Electronic' memberships are unable to to be processed without an email address.

All forms will open in a new window of your Browser (to save, right click on the link and select 'Save Target As' or 'Save Link As').

*These forms are sent to the Secretary, but you will need to send a printed copy of the form confirmation page with your original signature and payment enclosed. Prices and payment details are on the Membership form.  You may not enter an ADAA competition without having received your ADAA membership confirmation and registration details from ADAA.
